Add to this, if the quality of young players is random, randomness is probably programmed in a get.time() and similar functions manner. Now when one manager hits the right time for good quality he will likely get more than just one talented player at once.
If we combine this with the second theory from above the consequence is that there are a few teams pulling several good players at once, while others will get only weak players because the game engine tries to keep the quality prospects to consistent numbers.
Need another theories. I don't like the ones above.

Your second theory is more close to reality ! vlady stated some time ago how this youth pull works..
First of all, the average of youths' OR is higher when you have a high level Sports Academy. As you said, on level 5 you have pulled '140 overall players, all of them', which should not happen to a level 11 facility (I'm not talking about 1 player, but an average of all players pulled).
Now, about players' qualities ... These qualities are not influenced by anything, is just random ! Also, vlady stated that youths are generated according to a gaussian distribution. If you are not familiar with this term, I can tell you what it means (in PPM world).
Let's say that in one pull you have a total of 2500 players generated (all teams in PPM). What will be the qualities for the youths? Will be something like this:
AQ 1% - 1 player
AQ 2% - 2 players
AQ 3% - 3 players
..................
AQ 48% - 48 players
AQ 49% - 49 players
AQ 50% - 50 players
AQ 51% - 49 players
AQ 52% - 48 players
..................
AQ 97% - 3 players
AQ 98% - 2 players
AQ 99% - 1 player
It's just an example...
The idea for gaussian distribution is that most of generated player will have average AQs (around 50%), and only few of them will have vey high or very low AQs. Now, what will be chance that a player with 99% AQ (which I guess it will come only once at 3-4 pulls, in all PPM world) will be generated on your team ? How many teams are in PPM world ?
