Twice. Change position of both players. Although I´m not sure will work since they will be going to occupied position. Maybe Vlady can inform you better.

You need to do it twice, you cant just put LM in a place where currently RM is. You have to move RM somewhere than put LM into RM place and than put RM to the LM place
With a 5 min interval? I though about that. Seems nice idea to me and it will certainly work.
Another (needs more changes but would go faster and less dramatically) way would be
Change 1 RM to RM´ at min t
Change 2 LM to LM´ at min t
Change 3 LM´to RM at min t+5
Change 4 RM´to LM at min t+5

If RM´ is RMD or RMF (defensive or atackive) RM than that is what I had in mind.
Exactly that my friend, I failed to see first time I read you. Actually your method is more simple and produces same result.
No no, your method is actually the right one, because if you do it as I said, it can easly be a problem, because you dont know what subs are done 1st. So, its better 1st to move them both to a position RMD and LMD for example, and than, 5 minutes later change RMD to LM and LMD to RM.
Ofcourse in this exact example you need to have universal players on side midfields, otherwise they will perform bad when you change them sides...
