Why are you saying it hasn't been updated, if I can see 30% in the guide?
If it was Sarth he wasn't here for quite a while so maybe he isn't acquainted yet with new rules.
Honestly, I don't believe forums are read within 20% of entire PPM population, news with 40% and Guide 20%. I'm not defending anybody, just saying what could be the cause.

I believe the first offense is always a warning.
Yes, it could be a cause but ... they should be penalized ! The ones who had 2 teams (or the ones who broke any other rule) could say same thing

Yes and no.
The rule is only broken if BOTH teams are involved, as that's unsportsmanlike. If only 1 team is, then the other team is simply in the wrong place at the wrong time.
That said, in most games a warning is usually given for first offenses, or offenses where guilt can not be fully proven, so I do believe a warning should be pretty much what to expect.

You are probably right.
U cannot say that this example is not an understanding between managers. check the goal-keeper from the first game and u got your answer. 
I made a assumption after misreading mrk's message. My bad, 30% it is anyway.
My team will not perform better if they are warned or banned ! For me is same thing...
