I think countries with less than 20 active teams be put back into their Continent (e.g. Europe). In Europe there are 3 nations with ~15 teams who were given their own country by creating extra fake teams temporarily (Scotland, Moldova and Montenegro). Wouldn't it be better to have more active teams in a league by merging them back into Europe? Otherwise this encourages smaller nations to create fake teams to get a national team.
Is the fact of fake teams proven? (I believe you're talking about Scotland). As far as I see, there's nothing bad about wanting your national team. If you don't want to fall behind scots, you can advertise in your country a little bit- I believe it shouldn't be that hard to collect those 15 teams in a week or two 
The standard should be based on long-lived teams. Say 20 teams of 20 or 50 experience.
Maybe smaller countries could have a different league structure, such as 8 or 12 team leagues in a 1:2 pyramid. 8 teams could play each other 6 times (42 matches) with champions promoting and 7th and 8th relegating. 12 teams could play each other 4 times (44 matches) with champions promoting and 11th and 12th relegating.
And there is no reason to require a country to have a league in order to have a NT,

No there is no proof of fake teams, but which ever way you look at it the teams were created just before the deadline to make the numbers over 20 and they didn't last more than a week.
Advertising is made harder by not having a real national identity and playing in leagues that have lots of bots and managers that come and go.

I suggest to calculate tournament standings immediately after the games are played. also the team of the day should be calculated after the games. it's really irritating that we must wait until the next day...

If for some reason (suspension, injury, etc.) a player in the line up cannot play, the entire line up is changed into a 4-4-2 with the best (?) players of the team. Even if that means fielding 11 players that were not even listed.
I would like to suggest that if there is something wrong with the line up, that a substitute who is on the list takes his place. That way only 1 position is changed, instead of 11!

It should. But you have to set a substitute for his position.
unfortunately, it doesn't. although it works that way in hockey, and according to guide, should work here.

i had one actually. Not as a defender, but as a player without a specification. According to the rules he should have taken over the spot, but he didn't. That's actually the reason why i always have at least one player on the bench without a specification.
cards for national cup matches should be cleared when losing such a game, because it irritates in lineup because when knocked out it's useless information!
