Both infractions don't need to occur at the same time, just be called at the same time (same whistle that a goal was scored on).
Also, apparently if two penalties end at the same time, the captain chooses who gets to leave the box first.

EDIT: The Captain must inform the refs and the they inform the timekeeper before this happens. IE the start of the penalty.

Lol, I just noticed that China is not in the country list, guess they have to send someone at your office to regulate if you want to add it lol! You could be plotting against the government, who knows...
Probably the country list on the international page. China is in the list just not alphabetized correctly - the countries that start with Ch are placed between the countries starting with H and I. It's a bug I reported many months ago that's never been fixed.
League game today my goalie got pulled after the first when my team was leading 2-0 even though my tactics were as follows:
Trailing by... from 99-99
Leading by... form 99-99
Goals against: 4 or more
Save percentage: less than 50%
Won the game so not too upset but it could have been really bad, since my backup did end up letting in 3 goals.

You were leading 2-0? Very weird. Don't know what's going on there. If a dev doesn't comment I'd email them.
Check your settings in the other options on that screen. I had an option set weirdly in the "after 1st or 2nd period" and even though I had selected a different option (after 2nd period only), it still messed things up.

Likely a bug due to the fact all games went to OT
yup, seems like a lot of problems with the system here today, my goalie problem is the least of the devs worries right now
